Which CPT codes go on a therapy invoice?

Common ones are 90791 (intake evaluation), 90834 (45-min session), 90837 (60-min session), and 90847 (family/couples). Use the code that matches each session length and type.

Do therapists charge sales tax?

Mental-health and medical services are exempt from sales tax in essentially all states. You do not add sales tax to therapy sessions.

Can I bill no-show fees on a therapy invoice?

Yes. List late-cancellation or no-show fees as a separate line without a CPT code, since insurance does not reimburse them. State your policy in your intake paperwork.
