It is an invoice built directly from a time log, listing each work date, the hours spent, and the rate. It doubles as both your record of hours and the bill you send, making hourly billing transparent.
List overtime hours on a separate line at your agreed premium rate (often 1.5x). Keeping it distinct from regular hours makes the higher charge clear and easy to approve.
Many contractors round to the nearest 15 minutes (0.25 hr) for simplicity. Pick one method, apply it consistently, and state it on the invoice so clients know how time is tallied.
It is not required but strongly recommended for hourly work. An attached or referenced timesheet backs up your hours, speeds approval, and supports your records if the IRS or client asks.
